Lake Region Home Health offers quality nursing care to individuals in their home, allowing them to recuperate in the comfort and security of home and family.Our caring staff of dedicated professionals understand the importance of kind, individualized care.  We work with your physician who monitors all services we provide in the home.Home Health Care can postpone or eliminate the need for a nursing home stay.  We assist our clients in leading an active, independent life.    Your plan of care is developed by our staff with input by you and direction from your physician.
 

Any individual can be referred for Home Health Services by their physicians, family members, friend, or even themselves. A Lake Region Home Health nurse will meet with you to determine your needs and answer your questions.

Your care will be managed by a registered nurse who will be your consistent contact.  The RN will serve as a resource person to answer your questions and arrange the assistance you need. Home Health Aides are specially trained to meet the needs of individuals in their homes.

Lake Region Home Health is Medicare and Medicaid certified and is licensed by the State of Minnesota.  It is available to adults in Kandiyohi County and portions of Stearns County.
